New Patients

Your first visit to Lake Washington Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more.

There's also background information about our committed staff and our first visit procedures. You can also save some time by printing out and completing the patient forms in advance of your appointment.
 

The Day of Your Appointment

Please arrive and check in at our office 15 minutes before your scheduled appointment time. Please arrive early if you will be completing the patient intake forms in the office. Please plan ahead so that you are not late for your appointment. When patients are prompt, it is more likely that the doctor will be able to see you at your appointed time. Please call 24 hours in advance of your appointment if you need to cancel. A $50 no-show fee will be assessed to your account if you are not present for your appointment or if you call to cancel within less than 24 hours of your appointment time.

Patient Forms

Please read our Notice of Privacy Practices, which is standard patient privacy information. Next, print and complete ONE of the three Patient Registration Form that is appropriate to you. Print, read, and sign our Office Financial Policy to indicate you have agreed to our terms of payment. Finally, you can print and complete the Patient Health History Form which is necessary for your initial exam.

Choose and Complete One of These Forms

• Minor Patient Registration Form
  All patients under 18, or over 18 if still on parents' insurance. This is also the best form for young adults who are college age or otherwise still living with their parents and covered by their parents' insurance.
• Adult Patient Registration Form
  Patients over 18 and insured by self, spouse, or partner.
• Medicare Patient Registration Form
  Patients over 65 or disabled who have Medicare Part B coverage.

  Health history quesionnaire for patient exam.

Optional / Miscellaneous Forms

• Authorization for Release of Medical Records
  This form authorizes the release of medical records to or from our practice.
• Consent to Treat Minor Patient
  This form gives permission and reviews responsibilities for us to treat a minor patient without the presence of parent or legal guardian.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this Web site. From Bothell-area, I-405 southbound , the most direct route is to take Exit 17 toward NE 70th Place and turn left off the ramp at NE 72nd Place. The road you're on immediately becomes NE 68th Street, and then later, as it bends south, becomes Lakeview Drive. Turn right onto NE 58th Street. Turn left into our parking garage or use the free 4-hour parking on the street.

From Bellevue-area, I-405 northbound , take exit 14, merging onto WA-520 westbound, toward Seattle. Take the 108th Avenue NE exit, and turn right onto 108th Avenue NE. Turn left onto Northup Way. Turn right onto Lake Washington Boulevard NE. Turn right onto NE 58th Street. Turn right into our parking garage or use the free 4-hour parking on the street.

From Redmond-area, WA-520 westbound , take the 108th Avenue NE exit, and turn right onto 108th Avenue NE. Turn left onto Northup Way. Turn right onto Lake Washington Boulevard NE. Turn right onto NE 58th Street and make an immediate right into our parking garage or use the free 4-hour parking on the street.

From Seattle-area, WA-520 eastbound , take the Bellevue Way exit toward Lake Washington Boulevard, making a left off the exit ramp. Continue northbound on Bellevue Way as it turns into Lake Washington Boulevard. Make a right on NE 58th Street and then an immediate right into our parking garage or use the free 4-hour parking on the street.
