Insurance Information

Our office deals with many different insurance companies. There are thousands of plans for patients to choose from, and some can change benefit, co-pays, and deductibles throughout the year at any given time. In addition, some insurance companies will refuse to give out specific plan information for patients until after the treatment has been completed. Dealing with these companies can be difficult and very time-consuming. We do our best to provide the most up-to-date information on your plan, but ultimately, it is the patient's responsibility to know and understand their plan. As a courtesy, we do ask that you keep us informed of any insurance changes.

We will gladly file a claim on your behalf, or you may do so yourself. In fact, insurance companies process claims faster when they are submitted by patients themselves. Regardless, please be aware that the majority of insurance plans are limited and often only pay for a portion of any procedure.

Co-Pays

Most dental insurance companies offer plans that only pay between 40% and 80% of needed treatment. This is why we ask for your portion to be paid at the time of your appointment.

However, if your insurance pays less than what they initially estimated, even though you've paid your estimated portion, you are still responsible for the difference and will be sent a bill for that amount.

We Are a Preferred Provider

Dr. Cristoforo and Dr. Carmosino are preferred providers for Delta Dental Premier and American Dental Plan of Wisconsin. We are not in network for any others, but are able to still accept all plans excluding Badger Care and Medicare/Medicaid plans.
