Sport Physicals

Most schools require some sort of a physician's certification in order for your child to participate in a school-sponsored athletic program. Your child's sports physical will be conducted quickly and efficiently. If your child has had a recent physical at Kids & Teens Primary Healthcare, then an additional exam may not be necessary. Sports physicals are $100.00 or they may be covered by your insurance plan in some cases. Sports physical forms that are presented on the day of the appointment can be completed within 30 days free of charge. Expedited form completion will incur an additional charge.
