What to Expect

On the day of your child's first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. You may even fill out the forms below in advance and bring them with you on the day of your first appointment so that we can register your child in a more timely manner.

Please provide the following information when you arrive for your visit:

• Insurance cards
• CoPayment
• Complete immunization records
• Medical records from your previous physician

We are located on the second floor of the Advocate Good Samaritan Outpatient Center across from McCollum Park.

Bolingbrook Office:
We are located within the Boughton Park Medical and Dental Complex.
