Maurer Family Dental offers in-office dental membership care plans for our patients without dental insurance for use at our office. The focus of our dental membership care plans is on preventative care to help prevent problems before they start, and the plans help reduce the cost of treatment when needed beyond preventative care.
We offer four separate plans, and we will help you to select the plan that is most appropriate for you and your family.

Basic Child (<13 years old)
- Cost: $20/patient/month ($240/year)
- Includes: 2 regular exams and 1 emergency exam
- 2 regular cleanings
- All recommended x-rays
- Sealants
- Fluoride Varnish application (twice per year)
- 15% off any in-office treatment
- Nitrous oxide at no additional charge at treatment appointments
Basic Adult (13 years old and up)
- Cost: $25/patient/month ($300/year)
- Includes: 2 regular exams and 1 emergency exam
- 2 regular cleanings
- All recommended x-rays
- Sealants
- Fluoride Varnish application (twice per year)
- Take-home whitening trays fabricated for $75
- Whitening kit refills for $20
- 15% off any in-office treatment
Basic Adult Plus
- Cost: $30/patient/month ($360/year)
- Includes: 2 regular exams and 1 emergency exam
- 2 regular cleanings
- All recommended x-rays
- Sealants
- Fluoride Varnish application (twice per year)
- Take-home whitening trays fabricated for $75
- Whitening kit refills for $20
- 30% off any in-office treatment
Adult Periodontal
- Cost: $75/patient/month ($900/year)
- Includes: 2 regular exams and 1 emergency exam
- Up to 4 periodontal maintenance cleanings
- All recommended x-rays
- Sealants
- Fluoride Varnish application (twice per year)
- Take-home whitening trays fabricated for $75
- Whitening kit refills for $20
- 75% off scaling and root planing
- 45% off any in-office treatment
Maurer Family Dental Membership Terms and Conditions
- This is a dental office membership care plan, not insurance. This agreement does not provide health insurance coverage, including the minimal essential coverage required by applicable federal law. It provides only the services described herein. It is recommended that health care insurance be obtained to cover dental services not provided for under this in-office membership care agreement.
- This membership care plan only provides benefits at Maurer Family Dental and cannot be used at any other location.
- Membership cannot be combined with current dental insurance plans. Per the State of Illinois Public Act 101-0190, “Neither the patient nor the dental care provider shall submit a bill to an insurer for the services provided under an in-office membership care agreement.”
- Your account must have a zero balance prior to enrolling in a dental membership.
- A dental membership plan is not retroactive. The dental membership plan will become effective on the date of enrollment.
- It is the member’s responsibility to utilize the services within their plan year limit. Unused benefits will not be carried over or refunded.
- This plan is not transferable to another individual.
- A member can opt out of the plan for a full refund within 30 days of enrollment if no treatment has been provided.
- Services are based on a plan year. Eligibility remains active for 12 calendar months from the enrollment date. There are no waiting periods. Your membership can be renewed at the end of each plan year.
- No substitutions may be made for included services.
- Maurer Family Dental retains the right to interpret any program stipulations.
